About The Position

MDS Coordinator (RN) White Oak of Rock Hill is seeking an experienced and detail-oriented Registered Nurse (RN) MDS Coordinator to join our compassionate team. As our MDS Coordinator, you will play a critical role in the resident assessment process, care planning, regulatory compliance, and reimbursement accuracy while helping ensure exceptional outcomes for our residents. Position Summary The MDS Coordinator is responsible for coordinating and completing the Resident Assessment Instrument (RAI) process, including MDS assessments, Care Area Assessments (CAAs), and individualized care plans in accordance with federal and state regulations. This position works closely with the interdisciplinary team to ensure residents receive individualized, high-quality care while maintaining accurate documentation and Medicare reimbursement compliance.

Requirements

  • Current, active Registered Nurse (RN) license
  • Graduate of an accredited school of nursing
  • Minimum of one year of long-term care experience
  • Working knowledge of Medicare, PPS, PDPM, and Managed Care reimbursement
  • Thorough understanding of federal and state long-term care regulations
  • Strong assessment, organizational, communication, and leadership skills
  • Proficiency with electronic medical record (EMR) and clinical documentation systems

Nice To Haves

  • Previous MDS experience preferred

Responsibilities

  • Coordinate the interdisciplinary resident assessment and care planning process
  • Complete accurate and timely MDS assessments for admissions, readmissions, quarterly, annual, significant change, and discharge assessments
  • Ensure Care Area Assessments (CAAs) and individualized care plans are completed within required regulatory timeframes
  • Review and update care plans to reflect residents' current needs and goals
  • Coordinate and participate in care plan meetings and family conferences
  • Communicate care plan updates with residents, families, physicians, and interdisciplinary team members
  • Maintain and manage Medicare PPS, PDPM, and Managed Care assessment schedules
  • Ensure documentation supports assessment accuracy and reimbursement requirements
  • Participate in state and federal surveys and assist with regulatory compliance
  • Stay current with MDS regulations, reimbursement guidelines, and industry best practices through continuing education
  • Assist with staff education related to resident assessments and documentation
  • Utilize electronic clinical software to maintain accurate resident records
  • Perform additional duties as assigned by the Director of Nursing
