About The Position

The RN Assessment Coordinator (RNAC) coordinates the development and completion of the resident assessment in accordance with the requirements of this state and the policies and goals of Ephrata Manor. This is a full-time Monday through Friday exempt position and reports to the Executive Director.

Requirements

  • Must possess a current, active PA license to practice as an RN
  • Must have 2+ years of experience in a long-term care facility (SNF)
  • Must possess the ability to deal tactfully with others and the ability to lead and work harmoniously with other personnel
  • Must possess basic computer skills

  • Previous RNAC experience is preferred

Responsibilities

  • Conducts and coordinates timely development and completion of the resident assessment (MDS) including the implementation of triggers and care area concerns.
  • Coordinates and facilitates with the Interdisciplinary Care Plan Team to assure the timely development of a comprehensive care plan for each resident.
  • Revises care plans when required, involving residents and family as appropriate.
  • Assesses residents at admission/readmission for appropriate Medicare coverage or denial; ensures benefits are explained to residents and responsible persons.
  • Sends appropriate Medicare letters according to guidelines.
  • Works in conjunction with rehabilitation department and discharge planning team to monitor resident progress and plan for potential discharge.
  • Determines and collects appropriate medical information to be forwarded to regulatory agencies as requested (demand bill, expedited review, etc.).
  • Participates in clinical rounds/morning meeting as assigned.
  • Participates in the quality assessment and assurance program.
  • Participates in surveys, as requested.
  • Follows UCC Homes' resident care policies, code of conduct, corporate compliance program, and HIPAA guidelines.
  • Completes accident/incident reports and related documentation.
  • Maintains a safe environment for residents and staff.
  • Complies with infection control procedures and participates in drills as required.
  • Other duties as assigned and appropriate.
