RN, Ambulatory - Transplant Specialist

This position will focus mostly on the quality aspect of our transplant department including, maintaining the monthly score card, providing monthly updates and reporting out at the transplant committee. Summary The RN Transplant will be responsible for coordinating a focused and effective effort between the transplant center and its major patient referral sources (e.g. Dialysis Units and Nephrology Clinics) for renal transplantation. Job Description Will assume a case manager role in assuring that potential transplant recipients and donors will be afforded timely; efficient transplant education, work up, updates, and activation for transplantation when directed. Will be responsible for a designated caseload of patients during the pre/post-recipient and pre/post-donor transplant process. Aspects will include coordination of referrals, responding to roadblocks within the transplant workup process, and providing patient, family, and staff educational programming. Works to assure policies, procedures, and all regulatory requirements are met to assure the transplant program stays in compliance with various governing agencies. May provide outreach services to regional dialysis and nephrology programs.
Requirements
- Graduate from an accredited nursing program required, including, but not limited to, American Association of Colleges of Nursing (AACN), Accreditation Commission for Education in Nursing (ACEN), and National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA).
- Currently holds an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state.
- Minnesota RN license is required within six months of date into position.
- Obtains and subsequently maintains required department specific competencies and certifications.
Nice To Haves
- Bachelor’s Degree in nursing required, or in the process of acquiring with an estimated completion date within five years.
Responsibilities
- Delegate care, services, operational and managerial clinical activities for transplant candidates and recipients.
- Manage information for internal and external transplant databases.
- Coordinate the entire transplant process for assigned patients, including initial evaluation, organ procurement, admission, testing, education, treatment, discharge and post-transplant care.
- Maintain the monthly score card.
- Provide monthly updates.
- Report out at the transplant committee.
- Coordinate referrals.
- Respond to roadblocks within the transplant workup process.
- Provide patient, family, and staff educational programming.
- Assure policies, procedures, and all regulatory requirements are met to assure the transplant program stays in compliance with various governing agencies.
