About The Position

Responsible for evaluating and admitting patients into hospice care, ensuring they meet eligibility criteria and understand the services provided. This role involves conducting comprehensive clinical assessments, coordinating with physicians and the interdisciplinary team, and completing all required documentation accurately and timely. Serves as a key point of contact for patients, families, and referral sources—providing education on hospice philosophy, goals of care, and end-of-life support. The Admissions Nurse also facilitates a smooth transition into hospice by initiating care plans and ensuring all services are in place in different care settings. Assesses patients for hospice eligibility, coordinates admissions, and ensures timely, accurate documentation. They educate patients and families on hospice care, collaborate with the care team, and help ensure a smooth transition into services with a strong focus on compassionate, patient-centered care.

Requirements

  • Graduation from an approved school of professional nursing required
  • Licensed as a Registered Nurse by the Texas Board of Nurse Examiners required
  • Minimum of 3 years of professional nursing experience
  • At least 1 year in a leadership or mentorship role
  • Valid Texas driver’s license
  • Auto liability insurance
  • A safe driving record
  • CPR certification required
  • Strong computer skills including knowledge of and experience with Microsoft Office products
  • Experience with electronic medical records (EMR)

Nice To Haves

  • Previous hospice or palliative care experience strongly preferred
  • Knowledge of hospice federal and state regulations preferred
