Requirements

• Active Minnesota RN license in good standing.
• Strong OASIS and admissions experience.
• Ability to work independently and manage your time effectively.
• Confident communicator with patients, families, and providers.
• Reliable transportation for field-based visits.

Responsibilities

• Complete Start of Care (SOC) admissions and OASIS documentation.
• Perform comprehensive in-home assessments.
• Develop and initiate personalized care plans.
• Coordinate services and communicate with providers and caregivers.
• Ensure safe, smooth transitions into home health services.
