RN Admissions Nurse - Warwick Forest Long Term Care
About The Position
The Admissions RN works under the guidance of the Director of Nursing and reports to the Facility Administrator. The role promotes smooth transitions of care and supports the well-being and satisfaction of new admissions and their families. The nurse facilitates admissions by collaborating with hospital discharge planners to ensure potential residents meet admission criteria. As appropriate, the nurse may participate in assessing, planning, implementing, and evaluating nursing care and provide for the emotional and physical comfort and safety of new admissions and their families.
Requirements
- Registered Nurse (RN) license from the Virginia Department of Health Professions or licensure in a compact state, required upon hire.
- CPR/BLS certification from the American Heart Association, American Red Cross, or American Safety and Health Institute, required within 30 days.
- Valid driver's license required.
- Weekend shifts required.
Nice To Haves
- Two years of experience in long-term care or skilled nursing.
Responsibilities
- Maintain working relationships with providers and other medical personnel in support of quality clinical care.
- Assist medical personnel in ensuring potential guests meet clinical criteria for the care setting and that appropriate equipment is identified and available at the facility.
- Coordinate with discharge planners and Central Admissions regarding upcoming admissions.
- Review and complete admissions, including chart review, insurance review, face sheet, and follow-up with the resident or guest after admission.
- Evaluate admission readiness of potential residents or guests to help minimize readmissions to the hospital.
- Participate in the hospital interdisciplinary Plan of Care.
- Assess potential admissions in consideration of customer needs, including services, medications, and equipment, to ensure the facility can provide appropriate and efficient services.
- Complete the typical assigned workload within the scheduled shift.
- Demonstrate flexibility and offer assistance to the team.
