About The Position

This role involves admissions to the homecare division, requiring the completion of initial home health assessments for patients and their families to determine home care needs. The position includes providing a complete physical assessment and history of current and previous illnesses. The Registered Nurse will utilize the nursing process for planning, providing, and evaluating skilled needs. A comprehensive patient assessment, reflected in accurate OASIS scoring, is essential. The role also involves developing a care plan with goals based on nursing diagnosis, incorporating therapeutic, preventive, and rehabilitative nursing actions, and including the patient and family in the planning and revising process.

Requirements

Graduate of an accredited school of nursing.
Associates degree in nursing.
Registered as RN with New Hampshire’s Board of Nursing.
A minimum of one (1) year recent acute care experience.
Ability to exercise initiative and independent judgment.
Excellent verbal and written communication skills and competent in problem solving abilities.

Responsibilities
