About The Position

The Admission/Liaison Nurse is an integral part of the Interdisciplinary Team, working closely with the Medical Director to determine and facilitate hospice admissions. This role is primarily responsible for the coordination and execution of the admission process into Hospice care. The nurse will effectively communicate the Hospice concept to families and patients, develop the plan of care, and communicate patient/family care needs to the team. They are also responsible for training and educating patients/caregivers and establishing an appropriate frequency of disciplines to meet patient/families' needs. The position requires the ability to deal with human suffering while maintaining professional boundaries, flexibility to change schedule at a moment's notice, daily travel, and effective self-management of their schedule. Excellent clinical skills are essential, including the ability to identify problems, perform head-to-toe assessments, educate and consult with physicians, and execute orders to meet patient's needs.

Requirements

  • Current unrestricted AR RN license
  • BLS certification
  • Strong assessment skills with critical thinking and high level of problem solving within a healthcare facility
  • Strong documentation and communication
  • Ability to stay calm and cope with human suffering/emergencies
  • Use of personal vehicle
  • Valid driver's license
  • Current auto insurance
  • Excellent clinical skills

Responsibilities
