About The Position

The Admission Care Liaison is a DRIVING position supporting Ohio's Hospice South Region, covering the greater Miami Valley and surrounding areas. The schedule hours are day/evening shift from 11am-9:30pm with every 3rd weekend rotation and 1-2 holidays per year. Ohio's Hospice provides superior care and services to patients at their end of life journey, seeking individuals with a heart for hospice. Requirements

Graduate of an accredited nursing school
Associate of Science in Nursing (ASN)
Active Registered Nurse license in the State of Ohio without any board actions
CPR Certified
Computer skills sufficient to properly document services and care
Ability to drive during daytime, nighttime or inclement weather
Valid Driver's License with Safe Driving Record
State Minimum Automobile Insurance Coverage
Must be able to pass a background check
Must be able to pass a 10 panel drug screen

Nice To Haves

Bachelor of Science in Nursing (BSN) preferred
Two years acute care nursing experience preferred
Certification in Hospice & Palliative Medicine (CHPN) preferred; required within two years of employment (Eligible upon completion of 500 hours in last 12 months of hospice/palliative experience)

Responsibilities

Provide quality nursing care and coordination of hospice services.
Admit patients into the hospice program efficiently.
Provide the patient and family with the appropriate resources needed at the end of life.
Maintain a conversion rate of 75%.
Skillfully use the electronic medical record by accurate documentation at the point of care.
