About The Position

The Admission Care Liaison is a DRIVING position supporting Ohio's Hospice South Region, covering the greater Miami Valley and surrounding areas. The schedule hours are day/evening shift from 11am-9:30pm with every 3rd weekend rotation and 1-2 holidays per year. Ohio's Hospice provides superior care and services to patients at their end of life journey, seeking individuals with a heart for hospice. Requirements

  • Graduate of an accredited nursing school
  • Associate of Science in Nursing (ASN)
  • Active Registered Nurse license in the State of Ohio without any board actions
  • CPR Certified
  • Computer skills sufficient to properly document services and care
  • Ability to drive during daytime, nighttime or inclement weather
  • Valid Driver's License with Safe Driving Record
  • State Minimum Automobile Insurance Coverage
  • Must be able to pass a background check
  • Must be able to pass a 10 panel drug screen

Nice To Haves

  • Bachelor of Science in Nursing (BSN) preferred
  • Two years acute care nursing experience preferred
  • Certification in Hospice & Palliative Medicine (CHPN) preferred; required within two years of employment (Eligible upon completion of 500 hours in last 12 months of hospice/palliative experience)

Responsibilities

  • Provide quality nursing care and coordination of hospice services.
  • Admit patients into the hospice program efficiently.
  • Provide the patient and family with the appropriate resources needed at the end of life.
  • Maintain a conversion rate of 75%.
  • Skillfully use the electronic medical record by accurate documentation at the point of care.
