About The Position
CareOne Home Health is a locally owned and growing company seeking a full-time Admissions RN to serve patients in the central valley. The company's goal is to continually serve the community by providing high-quality, compassionate care in the intimate setting of patients' homes. Home health experience is required for this position.
Requirements
- Minimum one year nursing experience required
- One year Home Health experience required
- Knowledge of nursing care processes and medication guidelines/side-effects
- Committed to safety precautions and high ethical standards
- Proficient in reading/writing English
- Sound judgement and attention to detail
- Excellent communication skills and a compassionate nature
- Cool-tempered and patient
- Emotional/mental stability and stamina
- CA Registered Nurse license required
- Current CPR certification required
- Must have reliable transportation and auto insurance
Nice To Haves
- bilingual a plus (Spanish preferred)
Responsibilities
- Make initial visits to patients to complete comprehensive assessment and determine home health needs
- Collaborate with physicians and the Interdisciplinary Team to formulate and review care plans
- Make frequent visits to the patient to evaluate their condition
- Provide support and comfort according to individual patient’s needs
- Work with caregivers to plan and execute excellent patient care
- Notify a doctor for a patient’s condition when deemed necessary
- Assist patients and their families with paperwork
- Maintain accurate records
