The Data & Evaluation Lead for the Rural Health Transformation Program will lead the strategic development, implementation, and oversight of data systems, evaluation frameworks, and reporting structures for activities supporting the projected $1B (five-year project) federally funded North Carolina Rural Health Transformation Program (NCRHTP). Foundational to the program is the establishment of North Carolina Organizations Orchestrating Transformation for Sustainability (NC ROOTS) Hubs (Initiative 1). These locally governed, community-tailored networks connect medical, behavioral, and social supports, making it easier for rural residents to access comprehensive care in one place. This position oversees data reporting across all six initiatives including programs related to Chronic Disease, Physical Fitness, Food as Medicine (Initiative 2), Behavioral Health and Substance Use Disorders (Initiative 3), Workforce development (Initiative 4), Sustainable Financing with value base payment models (Initiative 5), and the expansion of Digital Health and Digital Literacy (Initiative 6) among rural providers and consumers. This position is responsible for building and maintaining integrated reporting structures that meet requirements from Centers for Medicare & Medicaid Services (CMS) while informing state-level decision-making.
