Requirements

High School Diploma and experience working in social services required.
Driver's license and current insurance required.

Nice To Haves

Bachelor's Degree and experience working in the field of social services with individuals with disabilities is preferred.
First Aid and CPR preferred.

Responsibilities

Assists participants with meeting employer expectations with work performance.
Provide training to improve participant's work performance after employer's training period has ended.
Problem solves to create accommodations or other ways to address performance issues.
Support participants to develop and maintain natural support and positive relationships with co-workers and supervisors.
Completes and submits agency documentation in a timely manner such as weekly progress notes and hourly billing.
Maintains participant file in accordance with agency requirements.
Utilizes time at participant's the work site to complete necessary documentation, when possible, maintains a minimum of 80% of hours worked in billable time.
Maintains knowledge of community resources that can be utilized to support participant needs (i.e., transportation, medical, childcare, utilities, etc.)
Makes referrals, advocates, and follows up.
Maintains confidentiality according to agency guidelines, follows ethical guidelines.
Completes training hours annually based on funded requirements.
Other duties as assigned.
