Requirements

  • High School Diploma and experience working in social services required.
  • Driver's license and current insurance required.

Nice To Haves

  • Bachelor's Degree and experience working in the field of social services with individuals with disabilities is preferred.
  • First Aid and CPR preferred.

Responsibilities

  • Assists participants with meeting employer expectations with work performance.
  • Provide training to improve participant's work performance after employer's training period has ended.
  • Problem solves to create accommodations or other ways to address performance issues.
  • Support participants to develop and maintain natural support and positive relationships with co-workers and supervisors.
  • Completes and submits agency documentation in a timely manner such as weekly progress notes and hourly billing.
  • Maintains participant file in accordance with agency requirements.
  • Utilizes time at participant's the work site to complete necessary documentation, when possible, maintains a minimum of 80% of hours worked in billable time.
  • Maintains knowledge of community resources that can be utilized to support participant needs (i.e., transportation, medical, childcare, utilities, etc.)
  • Makes referrals, advocates, and follows up.
  • Maintains confidentiality according to agency guidelines, follows ethical guidelines.
  • Completes training hours annually based on funded requirements.
  • Other duties as assigned.
