About The Position

This is an exciting opportunity to join a health system-based retail pharmacy operation within the IU Health Metro Region. This hospital-based retail pharmacy is located within IU Health West Hospital and supports the patient and employee needs for the growing campus in Avon, IN. The role provides comprehensive, value-based medication-related care to achieve definite outcomes that improve a patient's quality of life. The pharmacist acts in collaboration with other healthcare professionals in the shared responsibility for patient medication therapy outcomes.

Requirements

• Doctor of Pharmacy (PharmD) or Bachelor's of Science in Pharmacy prior to 2006
• 0-3 years of relevant experience
• Current Indiana Pharmacist license or eligibility for Indiana licensure as a Pharmacist
• Basic Life Support certification through the American Heart Association (AHA) may be required

Nice To Haves

• Successful completion of an ASHP accredited PGY1 or PGY2 residency may be required
• Board certification or other specialized training may be required
• May require travel to IU Health facilities
• May require weekend rotations

Responsibilities

• Ensuring safety and effectiveness of the medication use process
• Processing and dispensing medications per order or prescription
• Medication therapy monitoring
• Regulatory compliance
• Maintaining oversight over pharmacist extenders
• Maintaining the security of medications
• Protecting the confidentiality of patient information
• Providing medication information support for medical and nursing staff
• Ensuring continuity of care
• Identifying potential challenges and intervening, if necessary, to medication-related issues
