About The Position

The Restorative Aide position supports the provision of restorative and/or rehabilitative care for ProCare I-SNP members. This role involves providing personal care to patients in a manner that ensures their safety and comfort, and assisting in the provision of restorative or rehabilitative care. The aide performs restorative nursing tasks such as ambulation, range of motion, positioning, and self-feeding within the scope of their training, aiming to promote and support the greatest possible degree of independence for patients. The Restorative Aide advises the Director of Rehab (DOR) of the patient's status daily, contributes knowledge of patient conditions and needs to patient care plans, and assists in the implementation of care plans as directed within established policy and procedure. They also observe and report on patients' conditions and document these observations as required.

Requirements

State certification as a Nursing Assistant is mandatory.
Ability to read, write, and speak the English language.
Must be available to work six days per week.
Ability to travel to additional SNF locations within geographical area.

Responsibilities

Provide personal care to patients in a manner conducive to their safety and comfort.
Assist in the provision of restorative or rehabilitative care.
Performs restorative nursing tasks such as ambulation, range of motion, positioning, self-feeding, etc., within the scope of individuals' training.
Promotes and supports the greatest possible degree of independence for patients.
Advises Director of Rehab (DOR) of Patient's status daily.
Contributes knowledge of Patient's conditions/needs to patient care plans.
Assists in the implementation of care plans as directed within the parameters of established policy and procedure.
Observes and reports on Patients' conditions and documents observations as required.
