About The Position

The Restorative Aide supports restorative and/or rehabilitative care for ProCare I-SNP members as appropriate. The role provides personal care in a manner that supports patients’ safety and comfort and assists with restorative or rehabilitative care within the scope of the aide’s training. The aide promotes patients’ independence, contributes information about their conditions and needs to care plans, and assists with implementing care plans as directed under established policies and procedures. The position is based at Pivot at Park Valley Inn Health Center in Round Rock, Texas, and may require travel to additional skilled nursing facility locations within the geographical area. The schedule requires availability to work six days per week.

Requirements

State certification as a Nursing Assistant is mandatory.
Ability to read, write, and speak English.
Must be available to work six days per week.
Ability to travel to additional skilled nursing facility locations within the geographical area.

Responsibilities

Provide personal care to patients in a manner conducive to their safety and comfort.
Assist in providing restorative or rehabilitative care.
Perform restorative nursing tasks such as ambulation, range of motion, positioning, and self-feeding within the scope of training.
Promote and support the greatest possible degree of independence for patients.
Advise the Director of Rehab (DOR) of patient status daily.
Contribute knowledge of patients’ conditions and needs to patient care plans.
Assist with implementing care plans as directed and within established policies and procedures.
Observe and report on patients’ conditions and document observations as required.
