Requirements
- Current Iowa Registered Nurse (RN) license
- Strong communication and interpersonal skills
- Positive attitude and commitment to teamwork
- Basic computer proficiency and ability to learn work-related software
- Valid Iowa driver's license, reliable transportation, and auto insurance
Nice To Haves
- Bachelor of Science in Nursing (BSN)
- Two years of recent nursing experience, preferably working collaboratively within an interdisciplinary team
- Previous hospice, home health, palliative care, or related nursing experience
Responsibilities
- Respond to all calls received by the answering service on weeknights and provide direct hospice nursing care to patients.
- Complete assigned weeknight visits that are scheduled by primary care nurse.
- Consult with Physician for appropriate orders and communicate patient condition changes and the need for medical intervention.
- Uses teaching, assessment, observation, and intervention skills to provide comfort and care while maximizing patient/family quality of life.
- Manage the individualized patient plan of care and make necessary revisions as needs change.
- Participate in hospice admissions process as needed with accurate and thorough documentation.
- Work collaboratively with the Rest Assured team to ensure all on-call needs are met agency wide.
