We are seeking a highly skilled and compassionate Registered Nurse (RN) in the Albuquerque/ Santa Fe, NM area to join our team and make a meaningful impact on the lives of our patients. As a Resource Nurse, you will travel 2–3 days per week to support a variety of clinical responsibilities. This role is responsible for completing assessments for new, transferred, and current patients, as well as reviewing, developing, and coordinating individualized plans of care. You will provide education to patients, their families, and home health aides regarding medical conditions, disease progression, and care needs. The Resource Nurse will also support the onboarding and training of contracted caregivers. In addition, this position provides clinical support to the Director of Nursing, Compliance Manager, and the broader care team. Strong collaboration with patients, families, skilled and non-skilled caregivers, and interdisciplinary team members is essential to ensure effective care planning, service utilization, and overall patient outcomes. This role also includes connecting patients with appropriate community resources and recommendations.

Requirements

  • Active and valid RN license in the state of practice
  • Experience in Home Healthcare and knowledge of home health care plans, appropriate tasks/interventions to be completed by RNs, LPNs & Home Health Aides
  • Willingness to Travel to patient homes, including remote or rural areas in Northern New Mexico on an as needed basis if the patient/family is unable to present in office.
  • Ability to manage patient care in a home setting, with effective assessment and interventions
  • Strong Clinical Skills, including the ability to assess, plan, and manage patient care independently in a home setting

Responsibilities

  • Completing patient assessments
