About The Position

The Resource Navigator supports and serves youth and their families by providing and coordinating services and systems navigation to meet identified needs. This role involves responding to referrals in a timely manner, conducting strengths and needs assessments, and connecting families to community services by maintaining an understanding of state and federal offerings. The position requires sensitivity to the cultural and socioeconomic characteristics of the service population and direct collaboration with other organization personnel in an off-campus site.

Requirements

  • One year of case management or involvement in connecting families in federal or state service navigation (e.g., WIC, TANF).
  • Understanding of crisis intervention and trauma-informed care interventions.
  • Good verbal and written communication skills.
  • Working knowledge of basic computer skills.

Nice To Haves

  • Bachelor's degree in social work, psychology, or related human services field from an accredited college or university.
  • Bilingual skills.

Responsibilities

  • Provide and coordinate service and system navigation to meet identified needs of youth and families.
  • Respond to all referrals in a timely manner.
  • Conduct strengths and needs assessments.
  • Connect youth and families to needed services in the community.
  • Provide expedient and ongoing assessment, service navigation, case management, and transition planning for assigned families.
  • Maintain accurate and timely documentation of services for all assigned clients.
  • Prepare and submit all required documentation, reports, and statistical data in a timely manner.
  • Follow policies and procedures of ACH and represent the agency in a positive and professional manner.
  • Coordinate collaboration of referral agencies, family members, and other support systems.
  • Effectively communicate with agencies associated with referral agencies, complying with HIPAA and law.
  • Provide on-call support for Safe Place®.
  • Collaborate on a team of co-located service providers to address family needs.
  • Utilize expertise and awareness of state and federal services.
  • Partner with state and federal funded entities to connect families to services.
  • Establish formal Memoranda of Understanding between partner entities and ACH.
  • Collaborate with families utilizing a strengths and needs assessment to identify resilience and challenges.
  • Maintain effective engagement strategies to establish participation in needed service acquisition.
  • Provide families the opportunity to identify and address their own needs and ideas for overcoming challenges.
  • Facilitate and document assessment and service planning for all assigned program participants.
  • Document and assess program participant progress in a database.
  • Provide skills training and/or support groups.
  • Provide crisis response when necessary, assessing for safety and needed intervention.
  • Offer services after normal business hours, on holidays, and during assigned on-call rotation.
  • Participate in ongoing training and supervision.
  • Participate in staff meetings to share resources.
  • Provide support to families in developing discharge and aftercare planning.
  • Collaborate with other service providers and community organizations.
  • Represent ACH in a professional and positive manner.
  • Demonstrate awareness of community issues and priorities.
  • Maintain up-to-date case records.
  • Develop and document strengths and needs assessment.
  • Provide regular case management services for each assigned program participant.
  • Document case progress notes.
  • Communicate and coordinate needs for each client.
  • Assist in development of program related forms and measurement tools.
  • Complete a discharge and aftercare plan for each program participant.
  • Support and uphold the policies and procedures of ACH.
  • Maintain a positive morale.
  • Ensure quality services are provided and professional standards maintained.
  • Maintain an attitude of family, youth, and community advocacy.
  • Work harmoniously with co-workers.
  • Ensure work time is utilized fully and effectively.
  • Adapt to changing assignments/priorities.
  • Participate in team staffing meetings when necessary.
  • Participate in designated agency or community meetings or training.
  • Complete training and certifications as required.
  • Assist with other ACH activities as assigned.
