About The Position

We are seeking a Resource Coordinator to support our mission in empowering adults with intellectual and developmental disabilities. The Resource Coordinator (RC) serves as the primary point of contact for participants, families, and program staff, ensuring person-centered supports and services are delivered in alignment with individual goals and needs. This role manages a growing caseload while maintaining high standards of quality, consistency, and communication across services. This position supports participants across both our residential and day-program services, ensuring continuity of care and consistent coordination across all environments. In addition to core coordination responsibilities, this role includes a secondary focus on Crisis Response and Program Engagement, supporting participant safety, program stability, and a positive, inclusive environment.

Requirements

  • High school diploma or equivalent required
  • Minimum of 2 years of experience in developmental disabilities (DD) services, behavioral support, case management, residential services, or a related human services field
  • Familiarity with Colorado's Medicaid Waiver system (HCBS-DD, SLS)
  • Valid driver’s license and reliable transportation (if required for travel between program sites)
  • Ability to obtain and maintain CPR/First Aid certification
  • Must be able to successfully pass a background check and drug screen
  • Strong communication, de-escalation, and interpersonal skills
  • Ability to remain calm and effective during high-pressure situations
  • Strong organizational skills with attention to detail and follow-through
  • Collaborative mindset with a commitment to team consistency and accountability
  • Understanding of person-centered practices and positive behavior support strategies
  • Passion for supporting individuals with diverse needs and promoting inclusion

Responsibilities

  • Manage and coordinate services for approximately 40–45 participants, ensuring alignment with ISP, BIP (as applicable), and individual goals
  • Coordinate support for participants across residential and day-program services, ensuring consistency and continuity of care across all environments
  • Review, manage, and oversee Incident Reports (IRs) for accuracy, compliance, and follow-up
  • Communicate with participants and families regarding incidents, concerns, and service updates in a transparent and supportive manner
  • Serve as a liaison between participants, families, Program Managers, and Case Management agencies
  • Monitor participant progress toward goals and ensure timely, accurate documentation in internal systems
  • Support ISP meetings, annual planning, and ongoing collaboration with case management teams
  • Identify barriers to participation, safety, and goal achievement, and help develop solutions
  • Provide real-time support as part of the Crisis Response team during behavioral or emotional escalations
  • Collaborate with Behavior Team and Program Managers to ensure consistent implementation of crisis plans and debrief processes
  • Support participant engagement initiatives such as STAR of the Month and other recognition programs
  • Contribute to a positive program culture through encouragement, recognition, and morale-building efforts
  • Model calm, professional, and supportive behavior during high-stress situations
