Requirements

  • High School Diploma or equivalent
  • QMAP-certified OR expected to take part in QMAP training within 90 days of hire and to successfully pass QMAP certification.
  • Possess a valid Driving License (Minimum of 3 years)
  • Current Colorado Driver’s License
  • No license suspension or revocations within the last 5 years
  • No major motor vehicle violations in the last 3 years.
  • Clean driving record with no more than 2 moving violations in the last 3 years.

Responsibilities

  • Safely operate a client transportation vehicle over designated routes within an established time schedule.
  • Transport clients to medical, social, education and vocational appointments and on special event trips.
  • Conduct daily inspections of transportation equipment.
  • Resident supervision, de-escalation, medication monitoring.
  • Assist residents with daily needs and accompany residents on appointments and outings.
  • Assist with daily cleaning around the facility such as laundry, cleaning of communal areas, keeping the front desk clear of debris and clutter, sweeping, and mopping of lobby, vacuuming hallways and carpeted areas.
  • Conduct security audits including room checks, pat downs and urinalysis.
  • Follow mapping directions and adhere to assigned routes and schedules.
  • Assist passengers with boarding and exiting the vehicle.
  • Transport clients with special needs, physical handicaps, and high acuity mental illness to and from scheduled appointments.
  • Adhere to policies and procedures in maintaining appropriate client behavior.
  • Maintain the transportation vehicle in a clean and orderly condition and inspect and report mechanical defects or malfunctions promptly to supervisor.
  • Complete the daily transportation activity record, including the number of students transported, mileage, type of trip, and standby time.
  • Assist in accompanying residents with special needs, physical handicaps, and high acuity mental illness to and from scheduled appointments and outings.
  • Secure residents in seats.
  • Utilize appropriate de-escalation methods when appropriate.
  • Complete housekeeping tasks such as laundry, vacuuming, sanitizing, taking out garbage and keeping common areas free of clutter and debris.
  • Serve meals to residents.
  • Stock supplies i.e. paper towels, cleaning supplies.
  • Monitor client medication pass components.
  • Conduct client pat-downs, room searches and urinalysis testing.
  • Ensure timely and accurate client documentation.
  • Promote and demonstrate the Mission, Vision, and Core Values of Embrave.
  • Develop and maintain good working relationships with all departments, the management team, regulatory agencies, and community partners.

  • Responsible for the human, financial, and material resources as well as data and information entrusted to us.
  • Perform other job-related duties as required.
