About The Position

Vera French is seeking a compassionate and detail-oriented Residential Social Worker to support residents at our residential care facilities (Pine Meadows and Sheridan Springs). In this role, you will help develop and manage individualized treatment plans, provide individual and group support, coordinate services, and support residents as they work toward stability, and independence. This is a full-time position.

Requirements

  • Bachelor's degree in social work required; or bachelor's degree in a related social services field such as Sociology or Psychology with at least 30 semester hours in social work. Related experience may be considered in lieu of classroom training.
  • At least two years of experience working in a residential setting required.
  • Strong written and verbal communication skills, attention to detail, and effective organization and time management skills.
  • Ability to work well in a team environment and use Microsoft Office and electronic health record systems.
  • Health screening and TB test prior to employment with results that indicate the person is free of communicable diseases.
  • Maintain a valid driver's license and be approved by Vera French auto insurance carrier.
  • All Vera French employees that transport a Vera French client in his/her personal or agency vehicle must also have a D-3 Chauffeur's License. Employees are required to maintain a minimum personal vehicle coverage that equates to $100,000 each person and $300,000 each accident.
  • Must have no criminal record or founded dependent adult abuse report, or must be evaluated by DHS and given authorization for employment in a health care facility.
  • Satisfactory background checks with previous employers and references.

Nice To Haves

  • Master of Social Work preferred.
  • Experience in a state-licensed residential care facility preferred.

Responsibilities

  • Develop, implement, and monitor individualized resident treatment plans in collaboration with the interdisciplinary team.
  • Provide individual and group therapy and maintain communication with families and involved professionals.
  • Assess resident needs, write social histories and functional needs assessments, and document progress through monthly notes.
  • Coordinate discharge planning, referrals, aftercare services, and benefit-related support as needed.
  • Support resident programming, financial accountability, and staff training related to crisis prevention and resident care.
