About The Position

The Mary T Family of Companies is seeking a Residential Service Coordinator (RSC) for our ICF-licensed group home in Coon Rapids, MN. The ideal candidate will be a Certified Nursing Assistant (CNA) who is active and in good standing on the Minnesota Nursing Assistant registry, or that has medical coordination experience. The RSC is a direct care position that also provides technical assistance to the home Program Manager in order to create a healthy, safe and nurturing home for the residents. The RSC is responsible to assist the Program Manager to ensure the best care for the residents and that all care plans are implemented. In coordination with nursing, the RSC is to schedule and accompany the residents to medical appointments and communicate health concerns to team members. The focus of the RSC position is to assist in the day to day operations in the home. The RSC is expected to provide direct care, to lead by example, to have full knowledge of a resident’s preferences, needs and abilities, and to communicate regularly with the resident’s team members.

Requirements

  • CNA certification, preferred
  • Medical Coordination or experience, preferred
  • Valid driver's license and good driving record required
  • Previous Direct Support Professional (DSP) experience required
  • Previous experience in an ICF-licensed facility, preferred
  • High school diploma or equivalent

Nice To Haves

  • Certified Nursing Assistant (CNA) who is active and in good standing on the Minnesota Nursing Assistant registry
  • Previous group home experience
  • Familiarity with licensing rules and regulations
  • Some schedule flexibility to be able to meet the needs of the home
  • Ability to communicate with guardians, conservators and health care professionals

Responsibilities

  • Assist the Program Manager to ensure the best care for the residents and that all care plans are implemented.
