About The Position

The Qualified Disabilities Professional (QDP) is responsible for ensuring the development, implementation, coordination, and evaluation of person-centered services for individuals receiving residential and Medicaid Waiver services. The QDP promotes high-quality, individualized supports by ensuring compliance with Indiana Bureau of Disabilities Services (BDS) regulations, 460 IAC, CMS Home and Community-Based Services (HCBS) Final Rule requirements, and agency policies. The QDP serves as the clinical and regulatory resource for assigned programs by facilitating person-centered planning, monitoring service implementation, coordinating interdisciplinary services, identifying compliance concerns, and supporting continuous quality improvement. The QDP collaborates closely with the interdisciplinary teams to ensure services promote independence, health, safety, community integration, and meaningful outcomes for every individual supported.

Requirements

  • Bachelor's degree in Human Services, Psychology, Social Work, Special Education or related field required.
  • Meets Indiana Qualified Disabilities Professional (QDP) qualifications.
  • Strong assessment, documentation and organizational skills.
  • Ability to analyze regulations and implement compliant practices.
  • Excellent written, verbal and interpersonal communication skills.
  • Ability to facilitate meetings and collaborate with interdisciplinary teams.
  • Valid driver's license with acceptable driving record.
  • Ability to pass required criminal background checks.
  • Maintain CPR/First Aid and all required agency certifications.

Responsibilities

  • Ensure person-centered planning and service coordination promotes meaningful outcomes.
  • Develop, facilitate and maintain Individualized Support Plans (ISPs).
  • Complete annual, quarterly and other required service plan reviews.
  • Coordinate assessments and collect information necessary for individualized planning.
  • Ensure goals are individualized, measurable and outcome driven.
  • Monitor implementation of support plans and provide guidance when revisions are needed.
  • Evaluate progress toward desired outcomes and modify plans as appropriate.
  • Ensure services reflect each individual's preferences, strengths, interests and informed choices.
  • Assist individuals, families and guardians in understanding the planning process.
  • Ensure individuals actively participate in decisions affecting their services.
  • Ensure compliance with state, federal and agency requirements.
  • Monitor compliance with: 460 IAC regulations, BDS Medicaid Waiver requirements, Article 9 Client Rights, CMS HCBS Final Rule, BQIS standards, Agency policies and procedures.
  • Conduct routine record reviews and documentation audits.
  • Monitor implementation of rights restrictions, behavior support plans and risk mitigation strategies.
  • Ensure required documentation is complete, accurate and submitted within established timelines.
  • Identify compliance concerns and recommend corrective actions.
  • Participate in survey preparation, mock surveys and regulatory reviews.
  • Assist with development, implementation and monitoring of Plans of Correction.
  • Monitor corrective actions to ensure ongoing compliance.
  • Coordinate interdisciplinary services that support the individual's health, safety and quality of life.
  • Lead or participate in Interdisciplinary Team (IDT) meetings.
  • Collaborate with: Families and Guardians, Physicians, Nursing providers, Behavioral clinicians, Therapists, Vocational providers, BDS representatives.
  • Coordinate communication among service providers.
  • Assist with transitions between providers and services.
  • Promote access to community resources and meaningful community participation.
  • Advocate for services that support independence and person-centered outcomes.
  • Monitor quality of services through continuous quality improvement activities.
  • Conduct quality assurance reviews of assigned programs.
  • Monitor trends involving incidents, investigations, documentation and service delivery.
  • Review data to evaluate effectiveness of services and supports.
  • Recommend process improvements to enhance quality and compliance.
  • Monitor implementation of quality improvement initiatives.
  • Assist leadership with identifying systemic compliance risks before they become survey deficiencies.
  • Support agency performance improvement initiatives.
  • Provide consultation, education and technical support.
  • Serve as a subject matter expert regarding: Person-centered planning, HCBS Final Rule, 460 IAC regulations, Article 9, Medicaid Waiver requirements, Individual rights.
  • Provide consultation regarding implementation of support plans.
  • Educate Direct Support Professionals and management regarding regulatory requirements and best practices.
  • Assist with training related to person-centered planning, documentation and regulatory compliance.
  • Provide recommendations regarding complex clinical or behavioral situations.
  • Collaborate with Residential Managers to resolve implementation concerns.
  • Positively represent Easterseals.
