About The Position

Monitors and ensures that individuals are receiving quality/comprehensive care and services. Supervises and coordinates all personnel assigned to the home by completing various duties related to program planning, health needs, staff management, and regulatory compliance.

Requirements

  • Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.
  • Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.
  • Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form.
  • Ability to deal with problems involving several concrete variables in standardized situations.
  • Ability to think independently, gather relevant information systematically, grasp complexities and use problem solving skills daily.
  • Ability to act with clarity and in a timely manner.
  • Minimum of a bachelor’s degree in a related field.
  • 2 years experience working directly with persons with intellectual/developmental disabilities.
  • Experience in the facilitating and implementation of individual support plans which are person centered and meaningful.
  • Previous managerial experience required.
  • Demonstrated leadership ability.
  • Valid and current Pennsylvania Driver’s License.

Responsibilities

  • Participates in the referral process, including initial visits and orienting individuals and families/guardians to the home.
  • Completes required paperwork, including admission forms, assessment forms, and establishing individual records.
  • Coordinates individual program planning, ensuring monthly and quarterly documentation of participation and progress towards outcomes.
  • Develops, revises, and implements Individual Support Plans (ISPs).
  • Monitors individuals' health needs, schedules appointments, and ensures accurate paperwork completion.
  • Trains direct service workers on health and safety needs relevant to individuals.
  • Addresses immediate individual problems in collaboration with program staff and the interdisciplinary team, reporting serious concerns to the Director.
  • Supervises all personnel assigned to the home, including monitoring performance, training new staff, ensuring required training completion, conducting annual performance evaluations, and monitoring staff performance during announced and unannounced visits.
  • Coordinates monthly staff meetings with Program Supervisors to facilitate communication and ensure consistency of goals/outcomes.
  • Oversees the coordination of staff schedules, including assisting with substitutes and organizing schedules for efficient use of staff time and elimination of overtime.
  • Ensures coverage of staff shortages in the absence of a Program Supervisor or substitute staff.
  • Keeps Program Supervisors informed of agency happenings and changes, ensuring timely forwarding of communication.
  • Establishes a contingency plan for lengthy absences to ensure continuity of services.
  • Ensures compliance with agency policy and procedures, state regulations, county guidelines, and other relevant regulations.
  • Assures accurate completion of all required reports and forms, including medical reports, house budgets, individual expenditures, reportable incidents, programmatic reports, and payroll submissions.
  • Assists with internal licensing inspections and preparation for annual state licensing.
  • Acts as a host for visitors to the organization.
  • Demonstrates knowledge of emergency procedures and reporting requirements.
  • Serves on the Emergency On-call team, as a Certified Investigator, and as a Certified Medication Trainer or Practicum Observer at the discretion of the Director.
  • Conveys observations and key issues to the Director on a regular basis.
  • Attends and conducts meetings and training as required.
  • Completes a minimum of 24 hours of training relevant to human services training each training year.

  • Performs other duties and/or special projects as assigned.
