About The Position

Program Coordinators assist in overseeing the daily operations of the assigned program to ensure the quality of services provided to supported individuals. This includes, but is not limited to, managing the program’s schedule, payroll, and finances, as well as the confidential safekeeping and management of supported individuals’ personal information.

Eligibility for Employment
Ability to pass an initial criminal history check from the Department of Human Services Criminal Records Unit and recheck every 2 years thereafter.
Be a citizen of the United States or legally able to work in the United States.
Must be at least eighteen years old and hold a current driver’s license, insurance, and a good driving record.
Ability to complete required training and annual training thereafter.

HS graduate or equivalent required.
Two or more years of experience providing direct support to adults with disabilities, including hands-on assistance with Activities of Daily Living (ADLs) such as hygiene care, dressing, bathing, and continence care
At least one year of formal management or supervisory experience, including responsibilities such as coordinating program operations, managing program finances, ensuring policy compliance, and supervising employees (e.g., assigning tasks, providing feedback, overseeing performance, or participating in corrective action).

Responsibilities

Oversee the time, attendance, and performance of one or more Program Managers.
Interview, hire, and terminate staff as needed (in conjunction with the management team and HR).
Be responsible for the direct oversight and overall operation of the program(s) and ensure that the quality of life is in keeping with Home Life (HL) philosophy and values, policies, and procedures.
Maintain accessibility for staff to share concerns, problems, and grievances.
Share on-call responsibilities with the management team and provide direct support to supported individuals as needed.
Manage all community-related issues that arise in relation to supported individuals and staff (in conjunction with the management team).
Ensure that the Services Director and County are kept informed of all major issues that require follow-up or direct attention.
Ensure that all pertinent information regarding supported individuals or the program is communicated as needed, including supported individuals’ whereabouts, incidents that have occurred, and required follow-up.
Ensure that programs overseen are within the standards set by applicable OARs.
Maintain effective communication with the Services Director, Program Managers, DSPs, family members, and outside agencies as required.
Delegate responsibilities/work assignments to Program Managers and DSPs to ensure that all required tasks are completed.
Establish and maintain effective communication regarding the residential program with parents/guardians and supported individuals. Communicate medical, behavioral, and personal status changes or concerns to parents/guardians, as appropriate, in a timely manner while maintaining confidentiality.
Ensure that supported individuals are informed of their rights and responsibilities to themselves, others, and the home annually.
Coordinate with other programs as needed to execute company events and resolve problems.
Oversee coordination of staff schedules, including the scheduling of substitutes and volunteers, to ensure adequate coverage of the program. Meet weekly with the Program Manager to plan scheduling.
Attend weekly meetings with the Services Director and provide reports on program status.
Conduct weekly meetings with Program Managers and receive reports on program status.
Oversee regularly scheduled staff meetings, in conjunction with the PM, at least twice per month. Ensure agency agenda items and pertinent information are communicated to PMs and DSPs during staff meetings.
Attend all applicable program/agency meetings (staff, management, etc.).
Participate in the supported individual Intake/Exit process as needed, including all paperwork as requested by the Services Director.
Assist the ISP Team with information related to legal forms or issues as necessary.
Write and update Individual Support Plans (ISPs) for supported individuals as needed and attend all corresponding meetings.
Conduct and complete staff performance evaluations. Involve the PM in the process. (Review with the SD prior to presentation).
Oversee the evaluation and documentation of performance of DSPs routinely. Ensure there is follow-up training if a staff person is not performing their job according to acceptable criteria. Give timely positive feedback verbally or in writing if a DSP is meeting or exceeding criteria. Submit all written feedback to the SD.
Ensure that all department record systems are maintained at levels that meet required standards and policies.
Set up a notebook for each new supported individual within one week of their formal admission to the residential program. Perform monthly quality assurance checks to ensure maintenance of notebooks and Therap information.
Ensure that all supported individual notebooks and Therap IDFs are continuously organized, updated, and complete. Update supported individual medical, financial, personal inventory, personal data, and personal contacts yearly.
Review all program GERs and discuss incidents with PMs. Report GERs to the SD weekly and as necessary. Report incidents to the County DD Program.
Read all T-log entries and add follow-up to staff logs as needed.
Complete supported individual Monthly Change Reports, as needed.
Ensure that the supported individuals’ health and safety are maintained.
Oversee management of all program accounts, including Petty Cash, Activity Funds, Program/Food Budgets, etc. Perform mid-month and end-of-month cross-counts to ensure accuracy.
Complete any paperwork related to Social Security, Medicaid, or food stamps. Ensure that the appropriate documentation is maintained to complete this paperwork.
Review to ensure the accuracy of in-service records.
Ensure proper training and/or delegation of training of new staff, interns, and volunteers.
Ensure that staff are oriented to the program/agency.
Attend and facilitate in-service training activities as assigned by the SD.
Ensure that all staff in the program receive adequate in-service training each year and maintain their certifications in required training, e.g., CPR, FA, MART, BBP, Core Competencies, and OIS.
Oversee the design and implementation of supported individual training programs with appropriate staff.
Identify and communicate cleaning and maintenance needs to SD.
Assume additional responsibilities as assigned by the Services Director or Administration.
