About The Position

Hopewell Center is seeking a Residential LPN to assure optimal medical care for group home residents. This role involves assisting in establishing sound medical procedures for the residential department, with the potential for a hybrid work schedule for appropriate remote tasks. The LPN will coordinate medical appointments, communicate with physicians, monitor medication administration, implement physician orders, and provide follow-up care for residents' health needs. This position requires maintaining medical files and documentation according to state and Medicaid requirements, administering vaccines/tests, and providing health care training to agency personnel. The LPN will also respond to medical emergencies outside of regular working hours, participate in interdisciplinary teams and agency committees, and perform related duties to ensure the best interests of the agency and clients.

Requirements

Certified as a Licensed Nurse and possession of a current Indiana license in nursing.
Thorough knowledge of and ability to make practical application of PDR, State Board of Health, EDS/Medicaid Review criteria and other applicable regulations regarding health care.
Ability to successfully obtain certification in Core A & B (state-mandated medication administration curriculum) and TB skin test administration within required timeframes and maintain certification thereafter.
Ability to work with minimum supervision, both alone and with others in a team environment, often under time pressure.
Ability to work on several tasks at the same time.
Ability to read and interpret physician orders and lab reports.
Ability to maintain confidentiality of information and reports.
Ability to interact appropriately and communicate orally and in writing with other agency personnel, residents and their families/guardians and other health care providers.
Ability to conduct physical examinations of residents, including grasping and carrying objects, bending at the waist, crouching and kneeling to provide assistance to residents and/or lifting of residents.
Ability to observe behavior of residents and respond swiftly, rationally and decisively to emergency situations, despite possibility of exposure to potentially violent/irate individuals and situations involving potential physical harm to self and others.
Ability to work irregular and/or extended hours.
Ability to follow all personnel policies and rules of the organization.
Possession of a valid driver's license and a demonstrated safe driving record.

Responsibilities

Assure optimal medical care for group home residents.
Assist in establishing sound medical procedures for the residential department.
Coordinate scheduling and attend medical appointments for residents.
Provide ongoing communication with physicians regarding resident’s health care.
Monitor accuracy of medication administration, comparing physician orders with Medication Administration Record and prescription labels.
Implement physician orders completely and accurately.
Provide follow-up care and/or observation for residents' illnesses, accidents, and seizures.
Initiate, maintain, and provide follow-up communication with pharmacy to assure timely delivery of medications.
Maintain written summaries of nursing contact for each resident, including observation and chart review.
Maintain residents' medical files and documentation according to State Board of Health and Medicaid requirements.
Ensure electronic medication administration records are updated in real time.
Administer vaccines/tests as scheduled or as needed, including HEP-B and TB to staff and clients.
Schedule and assist in administering quarterly physical and medication reviews.
Provide initial and ongoing health care training of agency personnel pertaining to various medical techniques.
Provide initial and on-going training of state mandated medication administration curriculum.
Respond to medical emergencies and concerns that arise outside of regular working hours.
Participate in each individual's Interdisciplinary team and report on progress.
Participate on agency committees such as Human Rights Committee, Safety Committee, etc.
Perform related duties as assigned.
