Hopewell Center is seeking a Residential LPN to assure optimal medical care for group home residents. This role involves assisting in establishing sound medical procedures for the residential department, with the potential for a hybrid work schedule for appropriate remote tasks. The LPN will coordinate medical appointments, communicate with physicians, monitor medication administration, implement physician orders, and provide follow-up care for residents' health needs. This position requires maintaining medical files and documentation according to state and Medicaid requirements, administering vaccines/tests, and providing health care training to agency personnel. The LPN will also respond to medical emergencies outside of regular working hours, participate in interdisciplinary teams and agency committees, and perform related duties to ensure the best interests of the agency and clients.
