About The Position

Provides individuals in community-based programs with support services designed to promote independence, productivity, integration, and inclusion in the community. This position implements programs aimed at teaching new skills in the areas of self-care, managing behavior, community integration, and other activities of daily living. This is carried out with respect and dignity and in accordance with Walnut Street Inc. (WSC) policies and procedures.

Requirements

  • Follow age and population specific recommendations.
  • Understand and apply competencies specific to the general characteristics of each age group and specific population served.
  • Assist individuals with Activities of Daily Living.
  • Provide therapeutic environment.
  • Provide supervision to insure safety and well-being of individuals.
  • Provide supports that promote independence through skill building programs.
  • Encourage participation and be helpful, supportive and respectful of differences with regards to unique needs, age, gender and culture of the individual being supported.
  • Assist with the development, implementation, evaluation and documentation of individual’s treatment plans (ISP, DHSP).
  • Provide case management as assigned.
  • Assess individual’s skill set and areas of need.
  • Participate in the development of treatment plan and evaluate the effectiveness of the goals, objectives, plans and procedures.
  • Implement formal skills building programs, as identified in treatment plan.
  • Assist individuals to participate in health related activities according to WSC and/or DDS policy.
  • Assist in the development of Behavior Support Plan (BSP), and attend meetings as requested.
  • Manage behavioral crises.
  • Implement formal Behavior Support Plan (BSP) with support from the clinical team.
  • Apply crisis prevention and intervention strategies, as approved by WSC.
  • Manage both individual and groups in the program and the community.
  • Work effectively with more than one individual, encouraging participation in the program, providing support and offering assistance.
  • Ensure the appropriate ratios and level of supervision when interacting with individuals.
  • Promote individuals human rights by providing opportunities for choice and participation whenever appropriate.
  • Assist individuals in the day-to-day maintenance and cleanliness of the residence.
  • Ensure individual’s safe use of all equipment.
  • Ensure cleanliness, care and proper usage of materials, supplies and appliances.
  • Report deficiencies in materials, supplies and appliances to supervisor.
  • Provide transportation of individuals.
  • Operate house vehicle safely and defensively.
  • Follow all WSC transportation policies.
  • Promote a favorable image of individuals and the organization by fostering good community relations by demonstrating appropriate professional behavior.
  • Provide supports to ensure access to community, integrating opportunities for new experiences as appropriate.
  • Notify appropriate supervisor in the event of a community incident or any issue that could affect the individual, program or organization.
  • Maintain professional relationships with co-workers, management, guardians and members of the community in a professional manner.
  • Be responsive to constructive feedback and adjust accordingly.
  • Exercise good judgment.
  • Initiate course of action that follows WSC Policies, Procedure and Protocols.
  • Seek additional information from management if course of action is not established.
  • Resolve conflict in a productive manner and/or consult management for assistance.

  • Accurately complete in writing medical-related paperwork including, but not limited to: forms, monthly med charts, documentation of the administration of medication.
  • Assist with scheduling of medical and/or psychiatric appointments.
  • Attend appointments with individual.
  • Communicate verbally with medical staff, pharmacies and other medical professionals providing care to the individual.
  • Interact effectively and professionally in written and oral format with all of the organizations stakeholder; including but not limited to co-workers, management, guardians and members of the community.
  • Document and notify management of any issues brought forth that effect the therapeutic care or environment.
  • Complete all required programmatic documentation in an accurate and timely manner (ISP, BSP, medical, Incident Reports, recreation, logs, HR paperwork ect.).
  • Complete and maintain training programs/certifications as required as WSC, state and federal regulatory agencies.
  • Participate in continuous improvement to increase productivity, reduce inefficiencies and enhance the quality of services provided.

Responsibilities
