About The Position

In accordance with the organization’s mission, vision and values, the Residential Counselor is responsible for case management services. The following duties will apply: Responsible for the development of a comprehensive evaluation, phone screens, development and implementation of individual client service plans, ensuring that plans are initiated; provide follow-up and modification as necessary. Assure that record keeping is in compliance with agency and OASAS regulations and policy & procedures. Demonstrates teamwork and an ability to work in a team environment with coworkers and supervisors. Demonstrates the ability to be flexible and cooperative with the team. Completion of discharge activities according to policies, practices & procedures. Responsible for independent completion of case management duties. Ensure availability of and facilitation of the process for coordinating treatment referrals, funding/insurance referrals and insurance coverage. Responsible for providing counseling to residents in order to assist clients in the development of appropriate interpersonal, problem-solving and decision-making skills. Participate in general, staff, in-service and supervisory meetings held by the Agency or counseling staff. Assist clients in addressing issues and challenges by role-modeling appropriated responses. Assist in teaching the clients about personal care, household chores, daily living, community resources and safety awareness. Actively participate with the program team in completing proper daily maintenance of living areas including cleaning and maintaining an orderly environment. Assist with light cleaning. Assist clients with budget, grocery shopping and meal preparation as appropriate to treatment plan level of care. Maintain positive and effective relationships with community groups, referral sources and service agencies. Responsible for monitoring/distribution of residents’ medications. Perform other duties and functions as directed.

Requirements

  • Associates Degree in Health/Human Services
  • Valid NYS Drivers’ License
  • Excellent written and verbal communication skills.
  • Demonstrated ability to communicate effectively and work cooperatively with culturally diverse persons, staff and community service providers.
  • Knowledge of local behavioral health system.
  • Ability to multi-task, have good problem solving and time management skills and the ability to remain calm in crisis.
  • Proficient in Microsoft Word and Excel.
  • E-record.

Nice To Haves

  • Bachelors Degree preferred.
  • Pursuit of QHP stats is encouraged.
