About The Position

To coordinate and provide guidance, counseling, and encouragement to clients as they transition from the mental health system to a more independent environment. Assist the Manager with daily site operations and supervise direct service staff. Implement and document client progress in accordance with individualized rehabilitation service plans.

Requirements

  • Associate's degree in the Social Sciences, healthcare, or a related field and 2-3 years of relevant experience in the MH/MR field; or any combination of experience, education, or training that would provide the level of knowledge, skill, and ability required.
  • Good communication skills, both oral and written.
  • Ability to work independently and as part of a team.
  • Some knowledge of mental health systems and supports.

Nice To Haves

  • Prior supervisory experience is helpful.

Responsibilities

  • Provide support and ensure the delivery of appropriate services, reviewing client accessibility to these services.
  • Reinforce implementation of the individual rehabilitation service plans; incorporate modifications and updates, as directed.
  • Responsible for the operation of the site during assigned shift and/or in the absence of the Manager.
  • Participate in the resolution of crises; identify key individuals or resources that can provide direct support and/or immediate intervention during a crisis.
