About The Position

This is a full-time combined position serving the Breckenridge area in Wilkin County. The role includes two main components: Residential Coordinator (30 hours/week) and Mental Health Practitioner (10 hours/week). The Residential Coordinator ensures a safe, supportive, and enriching environment for individuals with disabilities in their home setting, overseeing daily care, managing team members and caseloads, providing direct care, ensuring regulatory compliance, and fostering independence and dignity. The Mental Health Practitioner provides Aftercare services to individuals transitioning from more-restrictive to less-restrictive living situations or those at risk of psychiatric hospitalization, offering general psychiatric rehabilitation and case management to enhance psychiatric stability.
