About The Position

The Transitional Support Program (TSP) is a Supported Apartment Program for Young Adults ages 18-25, staffed 24/7 by a multidisciplinary team. TSP focuses on teaching daily living skills, social and recreational skills, time management, interpersonal skills, conflict resolution, coping skills, and symptom management to help young adults transition to independent living. Services are provided through group and individual modalities. The Residential Counselor position is a team-oriented role that requires strong collaboration to meet client needs. Clients reside in shared apartments near the staff office. This role offers the opportunity to build impactful relationships with young adults and serves as a strong foundation for professional growth in the behavioral health field. Company vehicles are available on-site for client transportation.

Requirements

  • High school diploma or GED required.
  • Valid driver's license required with good driving record.

Nice To Haves

  • Two years’ experience working with a relevant population preferred.

Responsibilities

  • Maintains physical environment and program standards to CHR, DMHAS, JCAHO and any other licensing standard.
  • Monitors medications of all residents in accordance with the Adult residential medication policy and procedure.
  • Effectively utilizes appropriate communication methods/tools to transfer information to other staff regarding the day’s events.
  • Teaches and assists all residents in areas of rehab including: nutrition and healthy living, meal preparation, hygiene, laundry, banking and budgeting, and basic home care skills.
  • Assists/Manages administrative functions and tasks at the direction of the supervisor.
  • Prepares and facilitates educational and skill building groups and one to one interventions as directed by the treatment plan.
  • Collaborates and coordinates care with providers and families as needed.
  • Assists residents in addressing medical and psychiatric needs.
  • Maintains documentation in compliance with agency and program standards.
  • Participates in staff meetings, supervision, trainings and other meetings as assigned.
  • Additional duties as required.
