About The Position

This position is part of the Transitional Support Program (TSP), a Supported Apartment Program for Young Adults ages 18-25. The program is staffed 24/7 by a multidisciplinary team. TSP provides services focused on teaching daily living skills, social and recreational skills, time management skills, interpersonal skills, conflict resolution skills, coping skills, and symptom management skills, with the ultimate goal of helping Young Adults transition to independent living. Services are offered through both group and individual modalities. This role is a team setting that requires strong teamwork and collaboration to meet client needs. Clients reside in shared apartments with close proximity to the staff office. The position offers the opportunity to build relationships with young adults that have a lasting impact on their lives and serves as a great foundation for professional growth in the behavioral health field. Company vehicles are available on-site to transport clients.

Requirements

  • High diploma or GED required.
  • Valid driver’s license required with good driving record.

Nice To Haves

  • Two years’ experience working with a relevant population preferred.

Responsibilities

  • Maintains physical environment and program standards to CHR, DMHAS, JCAHO and any other licensing standard
  • Monitors medications of all residents in accordance with the Adult residential medication policy and procedure.
  • Effectively utilizes appropriate communication methods/tools to transfer information to other staff regarding the day’s events
  • Teaches and assists all residents in areas of rehab including: nutrition and healthy living, meal preparation, hygiene, laundry, banking and budgeting, and basic home care skills.
  • Assists/Manages administrative functions and tasks at the direction of the supervisor
  • Prepares and facilitates educational and skill building groups and one to one interventions as directed by the treatment plan.
  • Collaborates and coordinate care with providers and families as needed.
  • Assists residents in addressing medical and psychiatric needs.
  • Maintains documentation in compliance with agency and program standards.
  • Participates in staff meetings, supervision, trainings and other meetings as assigned.
  • Additional duties as required.
