About The Position
Fulltime position coordinating services for persons with disabilities. Set up and attend Person Centered Plan team meetings as required. Frequently checks with individuals in their residential and day settings to ensure their needs are being met. Be the point of connection between individuals and their natural supports.
Requirements
- Bachelor’s Degree in Human Services or a Bachelor’s Degree and prior experience.
- Must be able to pass a drug screening.
- Must be able to pass a background check.
- Must be able to pass a DOT physical.
