About The Position

This is a part-time (16 hours/week) position for a Residential Case Manager (Residential Counselor) within the Transitional Support Program (TSP) in East Windsor, CT. The TSP is a Supported Apartment Program for Young Adults aged 18-25, staffed 24/7 by a multidisciplinary team. The program focuses on teaching daily living skills, social and recreational skills, time management, interpersonal skills, conflict resolution, coping skills, and symptom management to help young adults transition to independent living. The role involves working in a team setting, building relationships with young adults, and contributing to their positive future. Company vehicles are available on-site for client transportation. This position is described as a great opportunity to build a strong foundation in the behavioral health field for professional growth.

Requirements

  • High diploma or GED required.
  • Valid driver’s license required with good driving record.

Nice To Haves

  • Two years’ experience working with a relevant population preferred.

Responsibilities

  • Maintains physical environment and program standards to CHR, DMHAS, JCAHO and any other licensing standard
  • Monitors medications of all residents in accordance with the Adult residential medication policy and procedure.
  • Effectively utilizes appropriate communication methods/tools to transfer information to other staff regarding the day’s events
  • Teaches and assists all residents in areas of rehab including: nutrition and healthy living, meal preparation, hygiene, laundry, banking and budgeting, and basic home care skills.
  • Assists/Manages administrative functions and tasks at the direction of the supervisor
  • Prepares and facilitates educational and skill building groups and one to one interventions as directed by the treatment plan.
  • Collaborates and coordinate care with providers and families as needed.
  • Assists residents in addressing medical and psychiatric needs.
  • Maintains documentation in compliance with agency and program standards.
  • Participates in staff meetings, supervision, trainings and other meetings as assigned.
  • Additional duties as required.
