About The Position

Horizon House is seeking a qualified candidate to join their Residential Service Group Home team in New Castle County, Delaware. This role involves providing coordination, guidance, counseling, and encouragement to participants transitioning to more independent living. The position requires implementing and documenting participant progress according to individualized rehabilitation plans and ensuring a safe, healthy, and secure environment that fosters growth and development. The role also includes completing assessments, developing recovery plans, assisting in case conferences, maintaining case records, liaising with community agencies and families, and assessing participant symptoms for timely interventions. The position requires attending new-hire orientation in Philadelphia during the first five (5) workdays (Mon-Fri), with mileage reimbursement provided.

Requirements

  • Bachelor’s degree in counseling, psychology, social work, nursing, vocational/psychiatric rehabilitation, education or other mental health field from an accredited college or university.
  • Knowledge of mental health and substance abuse systems and supports.
  • Ability to work independently.
  • Good communication skills both oral and written.
  • Possess a valid driver’s license with an acceptable driving record.
  • Direct experience in mental health services with emphasis on direct care in a community-based setting.
  • Ability to lift, carry, fold, bend, kneel, stack, stoop, which includes direct intervention with participants.
  • Frequent talking and listening.
  • Ability to climb and descend stairs.
  • Possess manual dexterity and fine motor skills.
  • Requires crisis intervention in emergency situations.
  • May be required to report to work during emergencies including inclement weather.

Responsibilities

  • Complete the initial and annual participant assessments and develop the participant’s recovery plan from a person-centered perspective.
  • Assist with the presentation in case conference for assigned participants.
  • Responsible for the care and maintenance of case records for all assigned participants.
  • Act as a liaison and consult with community agencies and families to maintain coordination in the recovery process.
  • Provide ongoing assessment of participant’s mental illness symptoms and their responses to recovery to ensure that immediate and appropriate interventions are provided.
  • Participate in staff planning, training and internal/external administrative meetings.
  • Implement and document participant’s progress in accordance with individualized rehabilitation plans.
  • Ensure a safe, healthy and secure environment which promotes growth and development.
