About The Position

Heartwood Home is a 12-bed, 3.1 level residential program for men aged 18 and older located in Willimantic, CT. It provides a structured recovery residence environment, staffed 24 hours a day, designed to prevent or minimize relapse or continued use. Residents have the opportunity to develop and practice interpersonal and group living skills, strengthen recovery skills, reintegrate into the community and family, and begin or resume employment or academic pursuits. Clients with co-occurring disorders and on Methadone maintenance are accepted. The length of stay is typically between 30-90 days, individualized based on medical necessity and clinical progress. The Residential Aide will maintain the physical environment and program standards, monitor medications, utilize appropriate communication methods to transfer information, teach and assist residents in areas of rehab (nutrition, healthy living, meal preparation, hygiene, laundry, banking, budgeting, basic home care skills), assist with administrative functions, prepare and facilitate educational and skill-building groups, collaborate with providers and families, assist residents in addressing medical and psychiatric needs, maintain documentation, and participate in staff meetings, supervision, and trainings. Additional duties as required.

Requirements

High School Diploma or equivalent.
Experience in Human Services with responsible work history.
Valid driver’s license required with good driving record.
CPR & First Aid Certification required OR must obtain within 90 days of employment.

Responsibilities

Maintains physical environment and program standards to CHR, DMHAS, JCAHO and any other licensing standard
Monitors medications of all residents in accordance with the Adult residential medication policy and procedure.
Effectively utilizes appropriate communication methods/tools to transfer information to other staff regarding the day’s events
Teaches and assists all residents in areas of rehab including: nutrition and healthy living, meal preparation, hygiene, laundry, banking and budgeting, and basic home care skills.
Assists/Manages administrative functions and tasks at the direction of the supervisor
Prepares and facilitates educational and skill building groups and one to one interventions as directed by the treatment plan.
Collaborates and coordinate care with providers and families as needed.
Assists residents in addressing medical and psychiatric needs.
Maintains documentation in compliance with agency and program standards.
Participates in staff meetings, supervision, trainings and other meetings as assigned.
Additional duties as required.
