About The Position

The Resident Wellness Coordinator (RPN) supports the Health and Wellness Manager in the delivery and coordination of resident care and wellness services within the residence. This role is responsible for providing professional nursing care to residents in accordance with established care plans, applicable legislation, and Chartwell policies and procedures. The Resident Wellness Coordinator provides clinical leadership and direction to front-line wellness team members, supports resident assessments and care planning, and assists in maintaining a safe and secure environment for residents, visitors, and staff members. This role acts as a liaison with residents, families, physicians, and external healthcare providers to support quality resident care and service delivery. All duties are performed in the best interest of residents and their families, and in accordance with Chartwell’s vision, mission, and values statements.

Requirements

  • Current registration in good standing with the applicable provincial nursing regulatory body (e.g., College of Nurses of Ontario) as a Registered Practical Nurse (RPN).
  • Current CPR/First Aid certification.
  • Medication administration certification or equivalent nursing education as required.
  • Minimum of two (2) years’ experience in retirement living, long-term care, assisted living, or a related healthcare environment.
  • Experience providing resident assessments, medication administration, and care planning.
  • Experience working with seniors and an understanding of geriatric care practices.
  • Strong clinical, organizational, and problem-solving skills.
  • Excellent interpersonal, verbal, and written communication skills.
  • Demonstrated leadership ability and sound clinical judgment.
  • Ability to work collaboratively within a multidisciplinary team environment.
  • Resident-focused with a commitment to service excellence and quality care.
  • Strong documentation and administrative skills with attention to detail.
  • Ability to prioritize workload and manage competing demands in a fast-paced environment.
  • Proficiency in Microsoft Office applications and electronic documentation systems.
  • Professional, respectful, and compassionate approach to resident care.
  • Ability to respond to emergency situations as required.
  • Ability to meet the physical demands associated with resident care activities.
  • Successful completion of a Criminal Record Check and Vulnerable Sector Check.
  • Proof of required immunizations and health screening as applicable.

Nice To Haves

  • Additional education in gerontology, retirement living, or senior care is considered an asset.
  • Experience supervising or directing front-line care staff is considered an asset.

Responsibilities

  • Assist in assessing, planning, implementing, and evaluating individualized resident care plans.
  • Provide direct nursing care and wellness support to residents as required.
  • Administer medications and monitor residents for changes in condition or response to treatment in accordance with professional standards and residence policies.
  • Monitor and update resident care plans, wellness documentation, resident profiles, and health records as required.
  • Review resident charts and ensure all required documentation is complete, accurate, and current.
  • Communicate changes in resident condition to physicians, families, and appropriate interdisciplinary team members.
  • Liaise with physicians, pharmacists, hospitals, community agencies, and external healthcare providers to support continuity of care.
  • Participate in resident admissions, transfers, discharge planning, and resident assessments.
  • Assist physicians during scheduled visits and ensure follow-up actions are completed.
  • Respond to resident and family concerns and escalate issues to the Health and Wellness Manager as appropriate.
  • Encourage residents to participate in therapy, wellness, and recreational programs that support quality of life.
  • Monitor residents receiving additional or enhanced care services and communicate changes or concerns as required.
  • Provide day-to-day leadership, supervision, and support to Personal Support Workers and wellness team members.
  • Assign and delegate responsibilities to front-line care staff in accordance with scope of practice and residence policies.
  • Monitor the quality of care and service provided by wellness staff and agency team members.
  • Assist with orientation, on-the-job training, mentoring, and ongoing support for wellness team members.
  • Promote teamwork and collaboration within the wellness team and across departments.
  • Participate in staff meetings, education sessions, and continuous improvement initiatives.
  • Contribute input into performance discussions and reviews for wellness team members.
  • Support residence policies, procedures, and standards of resident care.
  • Ensure resident incident/accident reports and required documentation are completed accurately and in a timely manner.
  • Report unusual occurrences, resident concerns, safety hazards, illnesses, and operational issues to the Health and Wellness Manager or General Manager.
  • Participate in quality assurance, infection prevention and control, and continuous quality improvement initiatives.
  • Assist in maintaining wellness supplies, medication administration records, and operational documentation.
  • Support emergency response procedures and participate in emergency preparedness activities and drills.
  • Maintain confidentiality of resident and employee personal information.
  • Participate in on-call responsibilities as assigned.
  • Conduct duties in accordance with Chartwell policies and procedures, professional nursing standards, and applicable provincial legislation and regulations.
  • Promote a safe working environment and ensure work is performed in accordance with Occupational Health and Safety requirements.
  • Report all actual or potential hazards, incidents, injuries, and safety concerns promptly.
  • Participate in fire safety, emergency code, and evacuation procedures as required.
  • Maintain knowledge of retirement residence regulations, nursing practices, infection prevention and control standards, and resident rights legislation.
  • Ensure resident dignity, privacy, confidentiality, and rights are respected at all times.
  • Build positive and professional relationships with residents, families, staff members, physicians, volunteers, and external partners.
  • Support a resident-centered environment focused on service excellence and quality care.
  • Act as an ambassador for the residence during interactions with residents, families, healthcare professionals, and community partners.
