About The Position

The Resident Wellness Coordinator helps manage our Wellness department to ensure the highest standards of care for our residents. This role coordinates nursing and care operations, manages Wellness team members, and ensures compliance with local, state, and federal regulations and community policies.

Requirements

  • Graduate of an accredited nursing program.
  • Valid PA/Compact LPN or RN license in good standing.
  • CPR certification required.
  • Minimum 2 years of long-term care experience with supervisory responsibilities.
  • Strong knowledge of nursing practices, senior living regulations, and documentation practices.
  • Ability to handle emergency situations calmly and competently.
  • Excellent proactive time management, attention to detail, organization, and documentation.
  • Strong leadership, interpersonal skills, and team development abilities.
  • Technologically proficient, especially in Microsoft Office and EHR software.
  • Strong verbal and written English communication skills.
  • Sound judgement and a strong ethical foundation in decision-making.

Responsibilities

  • Provide attentive, compassionate service that respects residents’ mobility, cognitive, and sensory needs.
  • Lead daily operations of the wellness department, ensuring quality resident care.
  • Monitor resident wellness statuses to ensure they are receiving the correct level of care and programming, facilitating transitions to other community programs as needed.
  • Collaborate with the Wellness Director, care team, and healthcare providers to determine and support individualized resident care plans, admissions, transfers, and discharges.
  • Oversee medication inventory and compliance with storage, counts, and handling regulations.
  • Ensure team maintains EHR, records, and logs with clear and accurate documentation.
  • Treat all residents, families, and team members with dignity, respect, and empathy.
  • Collaborate professionally and communicate clearly with healthcare providers, residents, families, and team members while maintaining confidentiality.
  • Ensure compliance with all health, safety, and regulatory requirements.
  • Support infection control and quality assurance programs.
