Requirements

  • Current Washington Nursing Assistant Certified (NAC)
  • Valid Driver’s License, Auto Insurance, and Safe Driving Record
  • 5+ years driving experience.
  • Current CPR Training.
  • Current Food Handler’s Card.

Nice To Haves

  • 1+ year experience in a social or recreational program in a health care setting preferred.

Responsibilities

  • Assesses, develops, and delivers a therapeutic activity program for an assigned case load including one-to-one interactions, group programs and trips/tours.
  • Delivers a program of scheduled activities as assigned, to include reality orientation, sensory stimulation, exercise classes, adaptive crafts, and pet therapy.
  • Plans, coordinates, and implements resident outings in eliseo vehicles and assists with special programs and events.
  • Assists with resident outings, special programs and events as assigned.
  • Maintains documentation required by State and Federal regulations including initial activity assessments (MDS), daily resident participation records, monthly, quarterly, and annual progress notes, and reviews.
  • Develops assessments and written care plans to promote non-pharmacological interventions and activity programs for an assigned case load including one-to-one interactions and group programs.
  • Provides support for in-house pet care.
  • Contributes to the maintenance and storage of supplies used in the activity program.
  • Provides transportation for residents on and off campus, including driving, scheduling, and escorting residents to assure safety between vehicles and pick up/drop off locations.
  • Communicates with Nursing Staff Coordinator and Unit Secretaries regarding appointments.
  • Assists Residents with personal care as needed during appointment transfer.
  • Performs transfers, lifting and positioning of residents to/from wheelchair, chair and vehicle assists residents with the use of walkers, wheelchairs, and canes.
  • Attends daily report meetings with nursing staff and participates in interdisciplinary team conferences.
