About The Position

Silver Tree Residential, LLC is seeking a dependable and experienced Service Coordinator to join their team at Christian Tower & Manor, a community of 155 apartment homes for seniors. The ideal candidate will be compassionate and motivated, dedicated to upholding the community's high standards and improving residents' quality of life. This role involves developing partnerships with local agencies, creating educational programs and events for residents, and understanding federal, state, and local resources for seniors. The Service Coordinator will act as a liaison with medical and professional services, report any instances of abuse, neglect, or exploitation, and maintain resident files in compliance with HUD and company requirements. While direct services are not provided, limited case management may be offered upon resident authorization. The role also includes developing a resource directory, assisting with resident transitions, collaborating on apartment inspections to support aging in place, creating monthly calendars and newsletters, and conducting home visits for prospective tenants to introduce the Service Coordination program and assist with move-ins.

Requirements

  • Minimum of a bachelor’s degree in gerontology, social work, or related field.
  • Minimum of three years of experience providing social services.
  • Genuine interest in continuing education that directly serves the elderly.
  • Informed as to legal liability issues relevant to the provision of social services to at-risk populations.
  • Sincere commitment and passion to serve an elderly population characterized by physical, functional, social, and cognitive challenges.
  • Strong written and verbal communication skills.
  • Ability to problem-solve, educate, and advocate for vulnerable adults.
  • Excellent organizational skills.
  • Knowledge of MS Office and experience with Excel spreadsheets is required.

Nice To Haves

  • Master’s degree and/or AASC Professional Services Coordinator certification is preferred but not required.

Responsibilities

  • Develop relationships and partnerships with multiple agencies and nonprofits in the area to help provide services for residents.
  • Link and develop ongoing educational programming, wellness presentations, and on-site events for residents that enable, empower, and promote resident self-sufficiency.
  • Serve as a liaison with medical and professional services providers, and community agencies upon request and/or authorization from the resident or their legal representative.
  • Report abuse, neglect, or exploitation of a resident in accordance with appropriate state statutes and standards of ethical practice.
  • Maintain resident files in compliance with HUD and Silver Tree Residential requirements, utilizing such files to prepare and submit HUD regulatory reports as directed.
  • Develop and maintain a resource directory for the apartment community.
  • Assist the resident, family, and designated representatives with transition into and out of the facility when consumer-directed.
  • Accompanies Community Director on apartment inspections, and collaboratively assesses and coordinates plan with residents to continue to age in place.
  • Follow up on any referrals from inspections from the property management team.
  • Create monthly calendar and site newsletter with input from the Community Director and other staff as needed.
  • Attend home visits of prospective tenants to engage in early communication about the Service Coordination program and assist in the move-in process.
