About The Position

Silver Tree Residential, LLC is seeking a dependable and experienced Service Coordinator to join their team at Fountain Place Apartments, a 102-unit senior community. The ideal candidate will be compassionate and motivated, dedicated to upholding the community's high standards and improving the quality of life for its residents. This role involves developing relationships with local agencies, coordinating educational programs and wellness presentations, and serving as a liaison with medical and community service providers. The Service Coordinator will also maintain resident files, develop a resource directory, assist with resident transitions, and create monthly calendars and newsletters. Additionally, the role includes accompanying the Community Director on inspections and conducting home visits for prospective tenants.

Requirements

  • Minimum of a bachelor’s degree in gerontology, social work, or related field.
  • Minimum of three years of experience providing social services.
  • Genuine interest in continuing education that directly serves the elderly.
  • Knowledge of legal liability issues relevant to providing social services to at-risk populations.
  • Sincere commitment and passion to serve an elderly population characterized by physical, functional, social, and cognitive challenges.
  • Strong written and verbal communication skills.
  • Ability to problem-solve, educate, and advocate for vulnerable adults.
  • Excellent organizational skills.
  • Knowledge of MS Office and experience with Excel spreadsheets.

Nice To Haves

  • Master’s degree and/or AASC Professional Services Coordinator certification is preferred but not required.

Responsibilities

  • Develop relationships and partnerships with multiple agencies and nonprofits to provide services for residents.
  • Link and develop ongoing educational programming, wellness presentations, and on-site events for residents.
  • Serve as a liaison with medical and professional services providers, and community agencies.
  • Report abuse, neglect, or exploitation of a resident in accordance with state statutes and ethical practices.
  • Maintain resident files in compliance with HUD and Silver Tree Residential requirements, and prepare HUD regulatory reports.
  • Develop and maintain a resource directory for the apartment community.
  • Assist residents, families, and designated representatives with transitions into and out of the facility when consumer-directed.
  • Accompanies Community Director on apartment inspections and collaborates on plans for residents to age in place.
  • Follow up on any referrals from property management team inspections.
  • Create monthly calendar and site newsletter.
  • Attend home visits of prospective tenants to communicate about the Service Coordination program and assist with the move-in process.
