About The Position
At Green Meadows Rehabilitation and Nursing Center, we are dedicated to delivering the highest quality of care to our patients and their families. We are seeking a compassionate and skilled Registered Nurse Assessment Coordinator (MDS) to join our team in beautiful Malvern, PA. As a Resident Nurse Assessment Coordinator (MDS), you will be responsible for processing related to the assessment of residents in our healthcare facility. This includes assessing patients, providing regular reports to doctors and other medical professionals, billing, and working with the MDS system.

Responsibilities
- Coordinate and complete all MDS assessments (admission, quarterly, annual, significant change, PPS, OBRA, and discharge) within CMS-mandated timeframes.
- Schedule and lead interdisciplinary team (IDT) care plan meetings; ensure individualized, resident-centered care plans are developed and updated.
- Accurately code MDS items to reflect resident acuity, ensuring appropriate PDPM
- Ensure compliance with all federal and state regulations, including OBRA, PDPM, and Quality Measures (QMs).
