The Resident Care Manager is responsible for providing leadership, oversight, and clinical coordination for resident care within the assigned unit(s). The RCM works closely with nursing staff, residents, families, physicians, therapy, social services, dietary, and other interdisciplinary team members to promote high-quality, person-centered care and ensure compliance with facility policies and applicable state and federal regulations.

Requirements

  • Graduate of an approved Registered Nurse/Licensed Practical Nurse program and licensed in the state of practice, required.
  • Previous experience as a Charge Nurse, Unit Manager, Assistant Director of Nursing, or Resident Care Manager.
  • Current BLS/CPR certification.

Nice To Haves

  • Minimum of 2 years of nursing experience in a Skilled Nursing Facility preferred.

Responsibilities

  • Provide leadership and oversight of resident care, ensuring services are delivered safely, timely, and in accordance with care plans, company policies, and applicable state and federal regulations.
  • Manage medication processes to ensure adequate supplies and that medications are properly handled, stored, administered, and documented in accordance with company policy and regulatory requirements.
  • Monitor residents for changes in condition and ensure timely notification of the attending physician, family member, or responsible party when significant changes occur.
  • Ensure incidents, accidents, adverse events, grievances, and resident concerns are promptly reported, thoroughly documented, investigated, followed up on, and appropriately addressed.
  • Promote and protect residents’ rights, dignity, privacy, independence, and choices while ensuring residents may exercise their rights without interference, coercion, discrimination, or reprisal.
  • Maintain the confidentiality and security of protected health information and ensure resident records and documentation are accurate, complete, and timely.
  • Collaborate with residents, families, physicians, nursing staff, and the interdisciplinary team while providing direction, coaching, and support to promote quality care, regulatory compliance, and positive resident outcomes.
