About The Position

We are Ohana Pacific Health, Hawaii's largest, locally owned post-acute healthcare company. Our vision is to transform how healthcare is provided throughout Hawaii. We are looking for a passionate Resident Care Manager / Licensed Practical Nurse who oversees the daily delivery of care of our ARCH residents. This role keeps the Director of Clinical informed of both progress and potential problems, coordinates community admissions, discharges, and transfers, and plans, develops, and organizes work tasks. The Resident Care Manager will coordinate ARCH services and activities in consultation with the Case Managers, plan and supervise resident care, continually evaluate residents' physical conditions, and participate in resident care conferences. This role also monitors quality indicators, ensures positive outcomes, actively participates in and supports continuous quality improvement efforts, and participates in admission and discharge planning as the ARCH Case Manager.

Requirements

  • Graduate of an accredited school of nursing.
  • Current Licensed Practical Nurse in the State of Hawaii required.
  • At least three (3) years' experience in a hospital or a long term care facility.
  • Current and valid 2 Step TB clearance and health review.

Nice To Haves

  • Supervisory experience preferred.
  • First Aid and CPR preferred.
