About The Position

Homestead Village Alabama is a compassionate assisted living community dedicated to providing exceptional care and support to our residents. We are seeking an experienced and motivated Resident Care Coordinator to oversee resident care services, supervise Resident Care Assistants (RCAs), and ensure compliance with medication management and assisted living regulations.

Requirements

  • High School Diploma or GED required
  • Computer proficiency required
  • Ability to lift and carry up to 50 pounds occasionally
  • Ability to exert up to 20 pounds of force frequently
  • Frequent standing, walking, bending, reaching, pushing, pulling, crouching, lifting, and repetitive motions
  • Ability to communicate effectively both verbally and in writing
  • Strong leadership and team management skills
  • Excellent organizational and documentation abilities
  • Effective verbal and written communication skills
  • Ability to work collaboratively with residents, families, physicians, and team members
  • Strong attention to detail and commitment to quality resident care

Nice To Haves

  • One to three years of direct resident care experience in an Assisted Living Facility (ALF) preferred
  • At least one year of experience in medical, surgical, geriatric, rehabilitation, or related healthcare settings preferred

Responsibilities

  • Train, mentor, and supervise Resident Care Assistants (RCAs)
  • Monitor staff attendance and maintain accurate scheduling records
  • Prepare and manage monthly staff schedules
  • Participate in the on-call rotation for resident care services
  • Assist with monthly resident assessments, including medication self-administration evaluations and medication cabinet audits
  • Ensure compliance with resident medication assistance guidelines and facility policies
  • Maintain and update weekly prescription logs and monthly medication records
  • Document resident care accurately and maintain nursing notes and records
  • Monitor residents for changes in physical, mental, or emotional condition
  • Notify physicians, family members, and administration of significant health changes promptly
  • Maintain knowledge of Alabama Assisted Living regulations and facility policies and procedures
  • Protect and promote resident rights, dignity, privacy, and independence
  • Support and monitor individualized resident care plans
  • Participate in care planning and provide recommendations based on resident needs
  • Respond to resident and family concerns with professionalism, empathy, and respect
  • Conduct resident satisfaction surveys
  • Maintain a professional appearance and adhere to facility dress code standards
  • Answer and direct telephone calls and inquiries appropriately
  • Supervise RCAs in assisting residents with medication management, bathing, grooming, and dressing according to established care schedules
  • Coordinate medication records
  • Conduct resident assessments
  • Communicate changes in resident health status
  • Work closely with physicians, families, and facility leadership to ensure high-quality resident care
