About The Position

The Resident Care Coordinator is responsible for the direct supervision of caregivers under the direction of the Wellness Director and administrator. The Resident Care Coordinator oversees all residents’ chronic medical condition assessment and management.

Responsibilities

  • Assist in the coordination of resident admissions.
  • Ensure each resident’s physician report is complete.
  • Complete a resident pre-assessment to determine appropriateness prior to admission, which may include visiting area hospitals, homes, or other assisted living facilities.
  • Prepare and complete a resident needs and service plan within 14 days after admission.
  • Review resident needs and service plans on a quarterly basis or as needed for any change of condition.
  • Communicate with the resident’s physician as needed.
  • Manage plans of action to correct deficiencies, and complete medical forms, reports, evaluations, studies, charts, etc.
  • Oversee all caregivers.
  • Ensure the facility medication room for all centrally stored resident medications is inaccessible to unauthorized personnel or residents.
  • Review resident medications to ensure there is a written physician order for each.
  • Maintain communication with the pharmacies to ensure all resident medications are managed.
  • Adhere to all Title 22 regulations regarding incidental medical and dental care.
  • Notify the resident’s family and or physician regarding any changes in condition.
  • Administer first aid as needed.
  • Acknowledge all residents’ rights and ensure staff does not violate those rights.
  • Ensure that any suspected or alleged elder abuse such as neglect, or mistreatment of residents is reported.
