About The Position

The primary purpose of this job position is to provide each assigned resident with routine daily care and services in accordance with the resident’s assessment and care plan, and as directed by supervisors. This role involves ensuring resident comfort, assisting with daily living activities, and maintaining a safe and warm environment. The position requires adherence to facility policies and procedures, accurate record-keeping, and effective communication with residents, supervisors, and inter-departmental personnel. The role also emphasizes maintaining resident confidentiality and reporting any changes in condition or incidents promptly.

Requirements

  • Must possess, as a minimum, a high school education or its equivalent.
  • Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.
  • Ability to write reports, business correspondence, and procedure manuals.
  • Ability to effectively present information and respond to questions from managers and employees.
  • Ability to apply concepts such as fractions, percentages, ratios and proportions to practical situations.
  • Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.
  • Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
  • Proficient in Microsoft Suite products.
  • Must occasionally lift and/or move up to 25 pounds.
  • Prolonged use of a desktop or laptop computer.
  • Regularly required to sit, stand, walk and talk, read or hear.
  • Frequent use of all office-related equipment to include; copier/scanner/fax, telephone, and calculator.

Responsibilities

  • Provide routine daily care and services to assigned residents according to their assessment and care plan.
  • Use PCC to ensure correct resident information when assisting with ALDS, meals, etc.
  • Report all changes in residents' conditions to the Health and Wellness Director promptly.
  • Record all entries on notes, SPAs, and incident reports in an informative and descriptive manner.
  • Report all observed accidents and incidents to the Executive Director and Health and Wellness Director.
  • Maintain resident confidentiality and promptly report suspected or known violations.
  • Report any known or suspected unauthorized attempts to access community's information systems.
  • Perform only trained care tasks.
  • Ensure resident rooms are ready, including making beds.
  • Greet residents, escort them to rooms, and introduce them to others.
  • Make residents comfortable.
  • Inventory, mark, and store residents' personal possessions and clothing.
  • Assist residents with packing for transfers or discharges.
  • Transport residents to new rooms or receiving areas.
  • Assist with loading/unloading residents to/from vehicles.
  • Create and maintain a warm, personal, and positive atmosphere, ensuring a calm environment.
  • Meet with nursing personnel to identify and correct problem areas and improve services.
  • Report all resident complaints and grievances.
  • Participate in and receive nursing reports.
  • Follow established policies concerning exposure to blood/body fluids.
  • Make beds (occupied and unoccupied) and add extra covers as requested.
  • Ensure residents unable to call for help are checked frequently.
  • Answer resident calls promptly.
  • Check residents routinely to ensure personal care needs are met.
  • Assist residents with identifying food arrangements.
  • Keep resident water pitchers clean, filled, and within reach.
  • Perform after-meal care (tray removal, cleaning hands/face/clothing).
  • Receive nursing reports upon reporting for duty.
  • Follow work assignments and schedules.
  • Cooperate with inter-departmental and facility personnel.
  • Notify the facility when late or absent.
  • Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals.
  • Assist residents with daily functions (dental/mouth care, bath, hair combing, dressing/undressing).
  • Keep residents dry by changing gowns, clothing, and linens when wet or soiled.
  • Assist residents in preparing for medical tests.
  • Assist with lifting, turning, moving, positioning, and transporting residents.
  • Perform restorative and rehabilitative procedures as instructed.
  • Assist in preparing residents for physical examinations.
  • Observe and report pressure areas and skin breakdowns to prevent bedsores.
  • Report injuries of unknown source, including skin tears.
  • Observe disoriented and comatose residents.
  • Record and report data as instructed.
  • Perform special treatments as instructed.
  • Immediately notify the Nurse of any resident leaving/missing from the facility.
  • Use only trained equipment.
  • Report defective equipment to the Nurse Supervisor/Charge Nurse.
  • Maintain confidentiality of resident care information, including protected health information.
  • Report known or suspected incidents of unauthorized disclosure of information.
  • Report all allegations of resident abuse and/or misappropriation of resident property.
  • Honor resident refusal of treatment requests and report such requests to a supervisor.
  • Give back rubs as instructed.
  • Assist residents with hair care functions (combing, brushing, shampooing).
  • Assist residents with nail care (clipping, trimming, cleaning).
  • Shave male residents.
  • Keep hair on female residents clean as instructed.
  • Position bedrest residents in correct and comfortable positions.
  • Assist residents with bowel and bladder functions.
  • Maintain intake and output records as instructed.
  • Keep incontinent residents clean and dry.
  • Check and report bowel movements and stool character as instructed.
  • Utilize protective and adaptive equipment as instructed.
  • Provide eye and ear care as instructed.
  • Weigh and measure residents as instructed.
  • Measure and record temperatures, pulse, and respirations (TPRs) as instructed.
  • Assist with the care of end-of-life residents.
  • Assist with post-mortem care as instructed.
