About The Position

Requirements

Ability to make responsible choices and decisions and act in the resident’s best interests.

Nice To Haves

Six months of experience in long term care or assisted living residence
Registered as a Nursing Assistant, Home Health Aid or Certified Nursing Assistant

Responsibilities

Provide hands-on care, both physical and emotional, as outlined in each resident’s service agreement.
Effectively communicate with residents and families, and notify the case manager of any changes in condition or concerns.
Assist in the dining room and participate in activities and outings.
Assist residents with ADL’s (Activities of Daily Living) as indicated on the individualized service agreement.
Help each resident to reach and maintain the highest level of performance and independence.
Provide residents with both physical and emotional support.
Assist with treatments as delegated by the Registered Nurse per the resident service agreement.
Observe clients and report changes in physical and emotional condition such as change in attitude, loss of appetite, weight, etc. to the case manager.
Interact with residents to provide reassurance, engagement, and redirection as appropriate.
Complete all required documentation daily, service schedule, flow sheet, communication book, etc.
