About The Position

To provide direct resident care services according to the plan of care prepared by and under direct supervision of the Charge Nurse. This role involves providing comprehensive care including nutritional support, continence care, mobility assistance, personal hygiene, comfort, and effective communication, while ensuring resident safety and well-being. The Resident Assistant will work in a well-lighted and well-ventilated environment, adhering to established policies and procedures.

Requirements

Must be at least 16 years of age.
Able to read, write and follow oral and written instructions and document care provided in English.
Must have successfully completed an approved NA training program and by on the Minnesota NA Registry or shall be enrolled in an approved training program and successfully complete that class and testing within 4 months of the date of hire.

Good Observation Skills-remains constantly alert to observe changes in resident’s status, recognized problems and hazards and report to the appropriate person. Uses observation skills prior to providing cares to determine if a licensed nurse needs to assess the resident.

Responsibilities

Provide direct resident care services with attention directed toward improving or maintaining optimal physical, mental, and psychosocial functional performance, promoting resident choice and preferences, and increasing resident dignity and quality of life according to each resident’s plan of care and the NA Standards of Care.
Perform nutritional care, including asking resident food preferences, serving meals, assisting with eating, encouraging adequate intake, recording intake, supplying fresh water, measuring weight, and reporting changes in appetite or intake.
Provide continence care, including supporting the use of bedpan, urinal, commode, or toilet, following bowel and bladder programs, collecting samples, maintaining urinary catheters, emptying and recording output, providing colostomy care, recording bowel movements, keeping residents dry and clean, cleansing after incontinent episodes, and reporting changes in elimination patterns.
Offer mobility assistance, including helping with ambulation, using mobility aids, transferring residents, maintaining joint functioning, repositioning residents, using proper turning techniques, encouraging activity, conducting range of motion exercises, assisting with rehabilitation programs, encouraging participation in therapeutic activities, assisting residents to and from activities, and reporting changes in mobility patterns or safety.
Assist with personal hygiene cares, including bathing, shampooing and hair care, skin care, grooming and perineal care, dressing and undressing, oral and denture hygiene, and nail and shaving care.
Address comfort and quality of life needs, including making decisions according to resident preferences, assisting with rest and naps, watching for signs of discomfort, reporting complaints, following care plan choices, assisting with socialization, exercise, and mental stimulation, maintaining a clean environment, changing linens, tidying rooms, and observing ventilation, temperature, light, and noise control.
Communicate effectively with residents, families, and other staff by communicating therapeutically and respectfully, providing compensation for diminished hearing, establishing eye contact, using low decibel sound levels, decreasing extraneous noise, responding to needs, reporting unmet requests, receiving shift report, communicating professionally between staff, reporting off duty, and reporting changes in communication patterns.
Provide for resident, employee, and GSLS protection by practicing basic medical asepsis, using correct hand-washing techniques, handling supplies and equipment properly, cleaning and sanitizing equipment, positioning call lights, using protective devices, observing call light policies, responding to call lights, observing fall prevention programs, knowing and practicing HIPAA privacy regulations, providing compensation for impaired vision and hearing, reporting hazards, observing self-back safety and body mechanics, following resident care plans, knowing and following established policies and procedures, knowing and following emergency and disaster plans, performing job procedures and skills correctly, being aware of safety, using special need safety devices, assisting with smoking, adjusting bed height, observing residents, reporting vulnerable adult issues, and reporting changes in resident status.
Perform other skills with special training, including taking vital signs, changing oxygen tanks, documenting care provided, vital signs, intakes and outputs, behaviors, and other duties as assigned.
