About The Position

This position involves providing direct care to residents, ensuring their physical, mental, and psychosocial well-being. The Resident Assistant will orient and mentor new staff, contribute to care planning, and assist residents with daily living activities while maintaining their comfort, privacy, and dignity. Responsibilities include accurate documentation, reporting changes in resident condition, and adhering to all facility policies and procedures, including safety and infection control.

Requirements

  • High School Diploma or equivalent.
  • Completion and proof of a PCA/HHA training program that meets the 54 training hour requirement from EOEA.

Nice To Haves

  • Certified Nursing Assistant certification preferred but not required.
  • Previous experience working in a long-term care/assisted living community preferred.
  • Previous experience working with dementia residents.

Responsibilities

  • Orients and mentors newly hired resident assistants.
  • Provides individualized attention to encourage residents' highest practical physical, mental, and psychosocial well-being.
  • Is knowledgeable of the individualized care plan for residents and provides support in accordance with their care plan.
  • Contributes to the care planning process by providing specific information and observations of resident needs and preferences to the nurse or care planning staff.
  • Attends to the individual needs of residents, including assistance with dressing, grooming, bathing, oral hygiene, feeding, incontinent care, toileting, colostomy care, prosthetic appliances, transferring, ambulation, range of motion, nail care, shaving, and communication.
  • Maintains the comfort, privacy, and dignity of residents.
  • Maintains the confidentiality of all resident care information.
  • Prepares residents for meals, serves food trays, and assists with feeding as indicated.
  • Records required documentation in the resident health record.
  • Reports all changes in resident’s condition to the nurse on duty/call as soon as practical.
  • Is responsible for promptly reporting any incidents or evidence of resident abuse or violations of resident rights to the nurse on duty/call or administration.
  • Observes and reports changes in resident’s physical conditions and cognitive/emotional status and maintains behavior log when indicated.
  • Conducts room checks and resident rounds.
  • Monitors for environmental safety hazards.
  • Records incidents and accidents.
  • Records resident and resident-related events.
  • Performs SAMM in accordance with facility policy and procedure.
  • Reports any discrepancy noted with family filled medi-sets and pharmacy filled calendar cards.
  • Ensures all prn narcotic medications are cued and signed off by two staff members.
  • Fills out SAMM Discrepancy Reports for medication refusals, out-of-medication situations, or any reason a resident does not receive medications as prescribed.
  • Documents all prn medications requested and their effect.
  • Honors resident's right to refuse care and reports such refusal to the nurse supervisor.
  • Assists in maintaining a safe, neat, and clean environment.
  • Ensures that residents who are unable to call for help are checked frequently.
  • Observes residents for changes in condition or behavior and promptly reports to the licensed nurse on duty.
  • Performs various tasks assigned by supervisor, including checking vital signs and weighing residents.
  • Assists with orienting resident and their families to the facility on admission.
  • Assists residents to walk with or without self-help devices as instructed.
  • Lifts, moves, and transports residents using proper body mechanics or lifting devices for accident prevention.
  • Uses only equipment trained to use and operates it in a safer manner.
  • Reports defective equipment to the nurse.
  • Communicates and interacts effectively and tactfully with residents, visitors, families, peers, and supervisors.
  • Assists and escorts residents to appointments such as the hair salon or attending activities, church services, or doctor’s appointments.
  • Performs all job responsibilities in accordance with safety and infection control policies and procedures, including thorough hand washing, use of disposable gloves where indicated, and proper disposal of soiled materials.
  • Follows established policies concerning exposure to blood/body fluids.
  • Attends and participates in mandatory and scheduled training and educational classes to maintain current certification as a Nursing/Resident Assistant.
  • Conducts all responsibilities in a manner that exhibits the IHC mission, vision, and core values.
